What is this service

This site is created to generate referral codes for prospective laser correction surgery patients to claim a £300 discount from the cost of their treatment operated by OXGO 2 LTD (Company No: 15571902). By using this service, the following Terms are agreed to.

Process

• A £300 Optical Express discount voucher is generated with the legal name provided within the form.

• The name provided must match that provided to Optical Express, a differing name may result in the voucher not being operational.

• A voucher with barcode will be sent to the email address provided, in the event of no delivery within 24 hours ensure the email address provided is accurate.

Data Handling

• No personal data is stored after voucher generation.

• Optical Express may retain and store your name for voucher validation.

Liability

• This system is not responsible if details do not match those provided to Optical Express, resulting in the voucher being rejected.

• Optical express have final authority over vouchers.

• This service has no other function after voucher generation.
